Our Services

Primary Care

Genevive has delivered primary care services to residents in long-term care facilities since 2003 and assisted living facilities since 2013. We work alongside patients, their families, and their facility to develop a plan of care related to their goals. All our primary care providers are board-certified and focused exclusively on geriatric medicine, supported by a specialized geriatric on-call group ready to respond when patients need us.

Transitional Care

Older adults with acute illnesses may not be well enough to return home when discharged from the hospital. They are often transferred to a Transitional Care Unit (TCU) at a nursing home - where the model of care should fit their special treatment needs. Our Genevive physicians and nurse practitioners assume care beginning the moment patients arrive.

Care Coordination

Genevive provides Care Coordination services for MSHO and MSC+ members, offering personalized support plans. Our Care Coordinators assess patients' needs, arrange services, and collaborate with healthcare providers. Key benefits include annual assessments, collaborative support plans, and seamless access to healthcare resources. Transition management services focus on patient safety, follow-up care, and reevaluating needed services.
