Population Health Nurse

We are looking for a Population Health Nurse to join our interdisciplinary care team! They will work with Genevive's high risk patient population to provide hands-on care in facility settings and utilize data from population health software to make treatment recommendations. This position requires active RN licensure, previous work with the frail and elderly population, and knowledge of value based care models.

It Director

We are currently seeking an IT Director to join our Leadership team. The IT Director will be responsible for the overall Genevive IT strategy, secure data management, and determines future IT needs. This position will also manage our relationship with our third party IT vendor. This position requires a bachelor’s degree, 5 years of IT experience, and knowledge of Microsoft programs. Previous healthcare experience is highly desired.
