Genevive has delivered primary care services to residents in long-term care facilities since 2003 and assisted living facilities since 2013. We work alongside patients, and their families, and their facility staff to develop a plan of care related to their goals. All our primary care providers are board-certified and focused exclusively on geriatric medicine, supported by a specialized geriatric on-call group ready to respond when patients need us.

Advanced Care Planning and Family Conferences
Genevive Providers work closely with their patients and their loved ones to understand what matters most during this stage in life. We focus on Advanced Care Planning conversations and offer family conferences, where we ask patients and their loved ones to tell us their stories about their life journey and identify specific, individual goals of care. In addition, we inform patients and caregivers about the purpose and safety of all medications, tests, and therapeutic interventions. This informed and collaborative approach ensures patients receive the care they want and deserve.

Physicians and Nurse Practitioners
Genevive physicians and nurse practitioners collaboratively work together to provide primary care to our patients. All Genevive providers have a focus and additional education in geriatric care. Our providers actively refresh their knowledge with the latest clinical evidence and share it with staff and families.

Specialized On-Call Group
Our specialized geriatric-trained physicians and nurse practitioners are available twenty-four hours a day, seven days a week. We work closely with facility staff to achieve in-time care for all clinical needs while focusing on the patient’s goals of care and personal wishes. We communicate with our primary team to follow up on clinical needs.

Helping Support Your Primary Care
Genevive provides a triage team of Registered Nurses (RNs) that are available for clinical needs and questions 7 days a week. The triage team is a liaison connecting your primary provider with facilities and families. Clinical documents are prepared and handled by a team of Genevive Licensed Practical Nurses (LPNs). Genevive also has a specialized team of Registered Nurses (RNs) who manage INRs for patients using warfarin anticoagulation therapy.
