Precepting Opportunities
To help emerging providers and clinicians grow their skills, precepting opportunities are available for Resident Doctors, Nurse Practitioners, and Physician Associates. We also offer shadowing experiences on a case-by-case basis.
Nurse Practitioner (NP) Full Time – South Metro
The Nurse Practitioner (NP) works as a member of the medical team to provide health care to individuals by evaluating health problems and instituting therapy in collaboration with the supervising physician. Within the Physician-NP relationship, Nurse Practitioners exercise autonomy in medical decision-making and provide a broad range of diagnostic, and therapeutic services.
Director of Population Health
The Director of Population Health will lead and advance efforts to improve health outcomes for frail older adults within a value-based care model. The Director will be responsible for the development and execution of population health strategies that integrate data analytics, community partnerships, and evidence-based care practices. This role will be pivotal in driving healthcare quality improvements and managing costs while ensuring personalized, high-quality care for our patient population.
MSHO Care Coordinator
We are currently searching for Registered Nurses or Social Workers to work with our growing geriatric MSHO or MSC+ member population. This position is responsible for assessing the medical and social needs of the members served to ensure all necessary care and services are established to optimize health and safety outcomes. They will work within the regulations set forth by MDHS and contracted health plans.
Triage RN
We are looking for a Registered Nurse with a strong medical background in geriatrics to join our telephonic Triage team. This position will support our providers by assessing patient condition and following provider approved protocols and make disposition or escalation decision efficiently and accurately. This is a full-time remote role with a 0.8 FTE and a every 3rd rotating weekend schedule.
Quality and Compliance Director
The Quality and Compliance Director will be responsible for the highest standards of patient care while maintaining strict adherence to regulatory compliance, particularly with Medicare quality regulations. This position will manage quality programs, collaborate with clinical leaders, and drive operational changes to improve patient outcomes. In addition, the role will integrate a focus on Social Determinants of Health (SDOH) and Diversity, Equity, and Inclusion (DEI), and ensure that quality and compliance initiatives promote equitable care for all patients, especially those from underserved or vulnerable communities.
