MSHO Care Coordinator - RN or LSW

Our MSHO Care Coordinators are responsible for assessing the medical and social needs of members in their homes and ensuring they receive all the necessary care and services. Working within the regulations set forth by DHS and Health plans for the MSHO program. We are looking to fill three caseloads within: Brooklyn Park, Plymouth, Andover, Blaine, Coon Rapids, Fridley, St. Paul, Minneapolis, Golden Valley, St. Louis Park, Robbinsdale, Hopkins, New Hope, and Crystal
