MSHO and MSC+ Care Coordination

Minnesota Senior Health Options (MSHO) and Minnesota Senior Care Plus (MSC+) are insurance products for individuals who are eligible for both Medicare and Medicaid. It integrates the benefits of both programs into a single health plan and provides an assigned Care Coordinator to determine the qualifying services and support each individual may need. Genevive provides compassionate Care Coordinators for patients enrolled in Medica, UCare, or Blue Cross Blue Shield MSHO or MSC+ insurance products and have a Genevive, Allina and Voyage Health primary care provider.

Genevive Care Coordination for MSHO and MSC+ Members

A dedicated Care Coordinator is assigned to each MSHO or MSC+ patient. Genevive Care Coordinators are either Registered Nurses or Social Workers and are certified assessors through the state of Minnesota. Their primary role is to collaborate with members in developing personalized support plans for each patient with the goal of optimizing their health outcomes and safety. We provide this beneficial care coordination in long term care facilities and in the community at assisted livings and patient homes. These services are aimed at coordinating care, transition management, managing chronic conditions, and ensuring that patients receive the appropriate healthcare services and support they need.

Assessments

We conduct annual in person visits to assess the patient's environment for safety and risks to ensure they are receiving appropriate care, services and support.

Support Plans

We collaborate with patients and their families to create a thorough person-centered support plan together, that is tailored to their individual needs and goals.

Arranging Services

We coordinate and arrange any necessary qualifying services that the patient may need, ensuring seamless access to healthcare resources.

Collaboration

We work closely with the patient's primary provider and other healthcare professionals involved in the patient's care, including during transitions of care.
