Providers use routine and acute visits to perform comprehensive medical evaluations. Our teams deliver compassionate primary care that is patient centered. We set shared goals and outcomes that preserve quality of live in a meaningful way.

Our team focuses on preventative and proactive care in your own home. We address patient medical needs, maintain functional status, and preserve a sense of wellbeing. The frequency of provider visits is based on the medical need of the patient and may vary from weeks to months between appointments.

We provide care to patients in TCU settings who need additional services to restore their health and wellbeing. Our provider team collaborates with therapies, dietary, social work and nursing staff to address the patient’s medical, functional and cognitive status to return to baseline health conditions.

Care Management improves patient care by reducing the need for unnecessary medical services. Genevive partners with UCare, Medica, and BCBS to provide various types of care coordination services. Care management staff are integral in helping members navigate the often complex health care system.

A cornerstone of the Genevive practice, the provider-led family conference engages the patient, family members, and providers. Our objective is to understand the patient’s journey through storytelling and have an in-depth discussion about the goals of care. This informed and collaborative approach ensures patients receive the care they desire and deserve.

Genevive is proud to foster partnerships with trusted health care organizations across the state who are committed to addressing the health needs of our geriatric population. We have strong community affiliations with Cassia, Presbyterian Homes and Allina Health. Genevive has partnered with Medica and UCare in offering specialized insurance plans focused on coordinating care for those who live in assisted living, memory care, and nursing homes.
